We have incorporated a new automated system to remind our patients that they are due for their eye health examination. This system will also confirm your appointment via text, e-Mail or with a telephone message. We will be asking you to confirm and update your contact information. We are very excited to initiate this automated system and as we merge the new with the old we may experience some technical difficulties. We appreciate your understanding if you receive multiple notifications or if we have to make accommodations to the schedule. Thank you.

- Identification
- Medical and Optical/Vision Insurance cards.
- Your eyeglasses, sunglasses, and computer glasses.
- If you wear disposable contact lenses, please bring the boxes, or the labels for both eyes.
- **For Children: 6-10 years old. To ensure a thorough and successful pediatric eye examination, we require the following prior to your child's visit:
- Previous eye exam records must be obtained and provided to our office prior to the visit.
- A child history form must be completed in advance. You may download and fill it out,or we can mail a copy to you upon request.
- If your child does not yet know their letters, please review the children's eye chart that we provide before the appointment to help familiarize them with the testing process.
- These steps are required so we can properly prepare for your child's visit and perform a comprehensive and efficient examination
